I understand I must submit this Vacation Request Form at least 4 weeks before the end date of the term before the requested vacation term. I understand I must pay a non-refundable deposit of $800 before my vacation is approved to hold my spot in the program. This is due 3 weeks before the end date of the term before the requested vacation term. I understand my current level and schedule may not be available upon return. The school will make efforts to place me in the best level and schedule according to the vacation policy. I understand that failing to follow proper vacation procedures may lead to F-1 status termination or suspension. I understand that I must maintain attendance of 80% or higher two terms before my requested vacation term.
